General School Admission Policies

 

 

 A. Requirement for Admission:


1. The following are to be submitted upon enrolling a patient:
A written diagnosis and referral letter from the following professionals:
Developmental Doctor/Pediatrician
Psychologist and Psychiatrist
Neurologist
Educators

2. A referral from any person can also be considered provided they have to accomplish requirement no. 1 above as soon as possible.

3. Medical records and/or educational records of all the professionals that had worked and still working with the child.

4. Interview of the client’s parents or guardians.

5. For transfer students, a copy of the most recent evaluation report by the previous teacher.

6. A medical clearance regarding the child’s participation in the center’s program or detailing whatsoever limitations in activities the child may have.

7. Needs assessment or evaluation to be conducted by GS educational specialist(s). This is a non-standard test done by using observations, school assessment and reports of information from parents.

B. Placement

As soon as a child/individual had passed the requirements and had been evaluated, he/she is admitted and placement program and services could be assigned by the program director.
